Health Insurers Ask for Hefty Rate Hikes

Some of the biggest health insurance companies in the country are proposing hefty bumps in the rates they charge customers. Regulators in some states can reject or downsize rate hikes -- setting the stage for a debate this summer on whether the Affordable Care Act is to blame for rising costs.
